The undersigned hereby affirms that the following statements are true and correct commencing with the date of this affidavit, and will continue to be true during my participation in this program:

  1. I am a duly authorized representative of the below designated Member or Prospective Member of the Foundation for Professional Pest Management (the "Foundation").

  2. I have read, understand and agree (on behalf of the Member or Prospective Member) to the professional standards adopted by the Foundation for designation of a pest control operator as a "QualityPro".

  3. I have also read, understand and agree (on behalf of the Member or Prospective Member) to the Foundation's Bylaws and the Membership Agreement.

  4. Based upon reasonable investigation, I have determined that the Member or Prospective Member complies with the following standards of the Foundation (as shown by a check added to the appropriate box):

  5. I certify that the service/sales employees of the Member (or Prospective Members) comply with the professional standards as established by the Foundation.

  6. Based upon reasonable investigation, I am unaware of any fact indicating that the Member or Prospective Member fails to meet the standards established by the Foundation for "QualityPro" status.

  7. I have attached hereto additional information (if any) that I believe the Foundation should be aware of in order to evaluate the Member or Prospective Member's status as a "QualityPro."
